Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept Cash or Check: Payment is due in full at the time of services. To receive a 5% bookkeeping courtesy, the entire balance for the treatment plan can be paid in advance at the first visit.
Visa, MasterCard, Discover and American Express: Payment is due in full at the time of service. We accept most major credit cards to allow you the most convenience in taking care of your account.
Financing
CareCredit / Wells-Fargo Financial: Payment is due in full at the time of service. We've made arrangements with a credit card company that will finance your dental treatment with approved credit. This will allow you to complete your dental treatment without delay and make relatively small monthly payments. Application forms are available in the office. Many of our patients take advantage of a 12-month, interest-free option. Visit www.carecredit.com for more info.
Gradual Treatment Plan
If you are on a tight budget, we can plan the completion of your dental treatment by spreading your appointments over a length of time to suit your oral health and your finances. We will arrange to begin the more urgent services as prioritized on your personal treatment plan.
Dental Insurance
Dental Insurance: Payment is due in full at the time of service. This includes services that may be covered by your insurance policy. As a courtesy, we will electronically file your insurance claim so that you can maximize your insurance benefits. Once the claim has been processed by your insurance company, you will be reimbursed directly by them.
